Should you wish to proceed with applying for a GA license, here are a few pointers that might be helpful:

• Enter your name and SSN. This will allow the portal to detect if you already have an application on file or are creating a new one
• If you are creating a new account, enter the required information and save your portal login information. You will need to log in to your account later in order to receive your GA license.
• Click on “Apply for a new license” on the left hand side navigation bar
• Select the following values at the bottom of the page:
  • Profession: Dietitian
  • License Type: Licensed Dietitian
  • Obtained by method: Registered (this means you are applying as someone who holds a valid CDR license)
• Complete the application form (link) and upload all documents via the online portal.
  • Note**:** You will be asked to fill out an employment verification form. Given Fay is NOT a W2 employer, please DO NOT list Fay as an employer and list your current and prior W2 employment roles in the form. You need to reach out to your current or most recent employer for verification on page 7 of the application.

Anytime you receive a new license or renew an existing one, please submit this form so we can keep your licensure information current.
