Should you wish to proceed with applying for an IL license, here are a few pointers that might be helpful:

• We've recently had dietitians obtain a license by filling out this form (please double check that it's still the latest version!)
• Given that you already have a different state license, your "method of licensure" will likely be "Endorsement of Licensure", but you would know best. If this is true, these 3 items are the only ones you need to fill out on the application
  1. Application: Pages 11-14 in the form above. There should be 4 pages total for the application.
  2. Supporting documents: Your current state license (step 1) and CDR license (step 8). Note that you may submit these two licenses in lieu of steps 2-7 on page 5.
  3. Fee: $100 application fee
  4. Submission address: Illinois Department of Financial and Professional Regulation, PO Box 7007, Springfield, IL 62791
