• Documents you will need:
  • CDR license
  • Official transcript from the college or university of your dietetic education - mailed direct to the Board Office from the University Registrar
• Complete the online application: link
  • Go to “Start new application”
  • Complete the account registration and hit “Create new application”. Please save this login information to retrieve your MD license later on.
  • Finish the form and submit payment online

Anytime you receive a new license or renew an existing one, please submit this form so we can keep your licensure information current.
