Should you wish to proceed with applying for an MA license, here are a few pointers that might be helpful:

• Documents you will need:

  • Items that must be uploaded to your application
    • Notarized CORI Form
    • Photo Headshot of yourself
    • Copy of your CDR Card
  • Items that must be mailed or emailed to the Board
    • Transcript of your highest-level degree related to Dietetics/Nutrition
    • Verification/Certification of all licenses you currently hold or have held

• Complete the online application here

  • Complete account registration by clicking “Create Account”.
  • Once signed in, click “Apply for a New License”
  • Scroll down to Board of Registration of Dietitians and Nutritionists and click “view more”
  • Finish the form, upload necessary documentation, and submit $196 payment online

Anytime you receive a new license or renew an existing one, please add them to the Credentials section in your Fay profile.
