Purpose

A practical, evidence-based learning resource for adult outpatient, telehealth-only MNT delivered by a registered dietitian. It supports individualized, stigma-free care across weight-neutral, weight-reduction, anti-obesity medication, and metabolic and bariatric surgery pathways.

How to Use This Guide

Use clinical risk, nutrition status, patient priorities, treatment history, readiness, and informed preference to select the care pathway. Weight change is one possible outcome, not a prerequisite for meaningful clinical benefit. This guide is not a substitute for specialty protocols, prescribing guidance, or individualized medical judgment.

Clinical Position

Obesity is a complex, chronic, relapsing disease influenced by biology, medications, environment, social determinants, sleep, stress, and behavior. Avoid explanations based on willpower or personal responsibility.

BMI is a screening measure, not a direct measure of adiposity, health, behavior, or individual risk. Interpret it alongside metabolic, functional, psychosocial, and nutrition findings.

Ask about the patient’s goals and preferred language around weight, and co-develop whether and how weight is used as a metric. Weight can be a meaningful, legitimate outcome to track when the patient wants to include it, on the same footing as other clinical measures; it should be neither imposed as the default focus nor avoided by default.

MNT supports comprehensive obesity care across lifestyle change, anti-obesity medication use, and metabolic and bariatric surgery.

Guide Outcomes

Scope Boundary

Medication selection, prescribing, dose adjustment, management of serious adverse events, diagnostic evaluation, psychotherapy, and surgical decisions are outside RD scope unless separately licensed or credentialed. The RD assesses nutrition risk, delivers MNT, and monitors response. Labs used in this guide typically come from what the patient uploads or reports. As a telehealth-only service, the RD does not routinely communicate with the patient’s prescriber and may do so only when the patient has authorized and arranged that contact.
