  • Click “Apply for a new license”
    • Select a Board: Medical Board
    • Select a License: Licensed Dietitian
    • Select an Application Type: General Application
• Documents required:
  • Completed background check through one of the options outlined here
  • Verification of CDR certification
• Application fee: $225.00 plus $3.50 transaction fee
