After your patient books their appointment, Fay automatically sends initial intake forms for you that were designed using best practices! Once these forms are completed, you will be able to review them in your patient’s profile. Most dietitians who want to create their own forms will send them via chat to their patients. There is currently no way to edit the automated intake forms.
You can access your patient’s completed intake forms and basic information in two ways:

Please note that the “View intake questions” button is at the bottom of the “Client Details” tab, and you will be able to view your patient’s answers once they complete the form.

Your patient will then share more information about their routines and habits.

Your patient will also be asked to share additional details about their medical and physical health.
