• Documents you will need:
  • CDR license
  • Copy of Drivers License, State Issued ID or Passport
  • Copy of Social Security Card
  • Verification of Lawful Presence
  • Official transcript from the college or university of your dietetic education - mailed direct to the Panel office from the University Registrar
  • State license verification
    • Contact the state board(s) where you are currently or have been previously licensed with and have the verification mailed or emailed directly to the Panel office.

  • Click “Click here to begin the electronic application”
  • Complete the account registration and hit “Create new application”. Please save this login information to retrieve your SC license later on.
  • Finish the form, upload necessary documentation, and submit $175 payment online
