• Documents you will need:

  • Have other RD state licensure information available
  • Clear copy of your valid CDR credentials
  • Drivers license information

• Complete the online application through the New Hampshire Office of Professional Licensure and Certification

  • **Create** or log into your account in the Utah Division of Professional Licensing Website
  • Click “Apply”
  • Select “Apply for a new license/ registration”
  • Select “Apply for a new Healthcare license/ registration”
  • Select “Dietitian”
  • Click “Start Application”
  • Select Profession: Dietitian, License Type: Dietitian, and Obtained By Method: RD Card after January 1 2002 as the License Type Selection
  • Complete the application form with your personal and professional information
  • Attach the required CDR document
  • Submit payment of the required $60 application fee online
