• Documents you will need:

  • Items that must be uploaded to your application
    • Signed copy of your current CDR registration card (RD/RDN)
  • Items that may need to be sent separately (if applicable)
    • License verification(s) from other states, if applying by endorsement
      • Endorsement: Request an official verification of license be sent to OPR for your initial state of licensure and most recent state of licensure. This must be sent directly from the licensing authority and can be sent by email to [email protected] or by post office mail to OPR at 89 Main Street, Montpelier, VT 05602.
      • Fast Track Endorsement: Request an official verification of license be sent directly to the Office. Verification must show proof of continuous licensure in good standing for 3 years preceding the date of this application This must be sent directly from the licensing authority and can be sent by email to [email protected] or by post office mail to OPR at 89 Main Street, Montpelier, VT 05602.

• Complete the online application through the Vermont Office of Professional Regulation (OPR)

  • Create or log into your account in the Vermont Online Licensing System
  • Select Apply for a License - License, and Dietitian as the profession
  • Based on your license status, select how you are applying for licensure (for more details you can refer to the official guide here):
    • Endorsement: If you have an active license in another state
    • Examination: If you don’t have an active license in another state
    • Fast Track Endorsement: If you have an active license in another state for over 3 years
  • Complete the application form with your personal and professional information
  • Upload required documentation
  • Submit payment of the required $115 application fee online
